Chelsea Bohac, AGNP-BC

Chelsea Bohac is board-certified in Adult and Geriatric Primary Care by the American Nurses Credentialing Center (ANCC). She is a seasoned clinician with over fifteen years of healthcare experience. Chelsea has worked in both acute care hospitals and outpatient practice settings. For the past 8 years, she has worked exclusively in Internal Medicine providing medical management and treatment for patients with a variety of complex health conditions.

 

Chelsea graduated Cum Laude with Highest Honors from the University of Hawaii in Honolulu, Hawaii, with a Bachelor of Science in Nursing. In addition to being awarded the Dean’s Research Award by the School of Nursing, she was also awarded Top Honors for the best overall research project and presentation at the University of Hawaii’s Spring Symposium.

While in Hawaii, Chelsea was an active volunteer with several community organizations, including the Waikiki Health Center’s Next Step Homeless Shelter, Friendly Neighbor program, and the Youth Outreach Clinic, demonstrating her strong commitment to both patient care and community service.

 

Following nursing school, Chelsea attended the University of Cincinnati where she earned her Master of Science in Nursing, specializing in Adult-Gerontology Primary Care. She completed competitive internships in Internal Medicine, Primary Care, Obstetrics and Gynecology, and Geriatric Medicine where she trained directly under experienced and skilled physicians.

 

Chelsea is passionate about teaching and has served as a Clinical Preceptor for nurse practitioner students from Texas State University, Houston Baptist University, and Chamberlain University. She is an active member of the American Association of Nurse Practitioners, Texas Nurse Practitioners, and Sigma Theta Tau International Honor Society of Nursing.

 

Outside of her professional life, Chelsea enjoys spending time outdoors with her family. Chelsea and her family are active members of the Katy community, where they proudly support local businesses, youth sports organizations, and the Katy ISD FFA.

      Rachael Tootle, FNP-BC

Rachael Tootle is a board-certified Family Nurse Practitioner (FNP-BC) through the American Nurses Credentialing Center (ANCC). With over 34 years of experience in healthcare, including the past 14 years as a Family Nurse Practitioner, Rachael has developed a broad expertise in both acute and outpatient care. She dedicated two years during the Covid-19 pandemic to exclusively working in mental health, providing compassionate care during a time of great need.

 

Rachael earned her Bachelor of Science in Nursing from the University of Southern Mississippi. She went on to complete her Family Nurse Practitioner training at the Frontier School of Midwifery and Family Nursing, where she specialized in Family Care. Her clinical training was conducted within the Coastal Family Health Center system in Biloxi, Mississippi, where she gained experience caring for underserved populations, including homeless individuals and victims of domestic violence.

 

Rachael has also contributed to the education of future nurse practitioners, serving as a Clinical Preceptor for students from Texas State University, Texas Tech University, and Texas Woman’s University. She is an active member of several professional organizations, including the American Association of Nurse Practitioners, Texas Nurse Practitioners, and the Houston Area Nurse Practitioners.

 

Outside of her professional life, Rachael enjoys traveling, baking macarons, and spending quality time with her husband, Jeff, and their college-aged children, Gabe, and Katie. The Tootle family has been an integral part of the Katy community since 2011, where they remain active residents.
